• Affiliated Law Firm Disclosure

Explanatory note: The form found below was developed by the Special Commission on Multidisciplinary Practice and Ancillary Business. The commission has been charged with enabling lawyers to participate in business ventures not engaged in the practice of law in conformance with The Rules Regulating The Florida Bar, through research and development of business models, prototype forms, ethical guidance and educational programs. The disclosure form can be used in connection with a lawyer’s ancillary business to inform customers of the ancillary business who are not legal clients that they do not have an attorney-client relationship with the lawyer who owns the business by virtue of being a customer. This form is not required in order to comply with a lawyer’s ethical obligations while engaging in an ancillary business. Any lawyer is free to modify this form to suit the lawyer’s individual needs and concerns.


Affiliated Law Firm Disclosure


S&J Consultants, Inc. has a business relationship with the Law Firm of Smith & Jones P.A. (the "Firm"). [Describe ownership interest/economic benefit.]

If you become a client of the Firm, it will not affect your relationship with S&J Consultants, Inc. Your continued dealings with S&J Consultants, Inc. will not be subject to the protections of the attorney-client relationship such as confidentiality and avoidance of conflict of interest.

By signing below, you agree that you have read and understand this disclosure and that it was provided to you prior to your decision to use the Firm.

Acceptance

I have read and received a copy of this disclosure and understand that my relationship as a customer of S&J Consultants, Inc. does not make me a client of the Firm, that the Firm may represent clients with interests adverse to mine, that the Firm may decline to accept me as a client and, that my use of the Firm does not alter my relationship with S&J Consultants, Inc.
